New Clients

If you would like to request a username and password to access your physician/practice specific results, please follow the instructions below:

1. In accordance with the Health Insurance Portability and Accountability Act (HIPAA) of 2003, it is required for all clients to print, sign, and return the following Notice of Privacy Practices Acknowledgement form electronically or by fax.Privacy Policy
2. Please provide us with a list of each individual that will require access via phone, fax, or email, including the following information:

• First and Last Name
• Requested Username
• Requested Password

NOTE: Be sure to fill in all required fields.

In addition, clients can order requisitions by phone, fax, or email as listed to the right.

Transport of Routine Specimens

All specimens are to be transported in a closed container in 10% Formalin with a pathology requisition and a face sheet.

NOTE: Formalin containers are provided for any specimen submitted to NTPL or NP.

Transport of Fresh Specimens

Please call 972-526-0300 in North Texas or 936-568-3181 in Nacogdoches at your earliest convenience for the following fresh specimens:

• Cytogenetics
• Flow Cytometry
• Frozen Sections
• Molecular Testing

Documentation of Specimens

Requisitions
Please provide the following required information:

• Date
• Patient’s First and Last Name
• Patient’s Sex
• Patient’s Date of Birth
• Select Party to be Billed
• Billing Party’s First and Last Name
• Patient’s Specific Billing Information

Container

Please provide the following required information:

• Date
• Patient’s First and Last Name
• Patient’s Date of Birth
• Specimen Type
• Physician’s First and Last Name

Request Pick Up

Please contact North Texas Pathology at 972-526-0300 or Nacogdoches Pathology at 936-568-3181 for courier service if a daily pick up is not arranged.

NOTE: Please contact us by phone for any specimen submitted after 4:00 PM.
